Event Check List

In an effort to better serve you and your ministry needs, please refer to this checklist as a guide to help you determine which forms you need to complete.


I am planning an event- Complete the Special Events Request Form.
My approved event requires financing- Complete the Event Budget Approval Form
I need to reserve a Room or Space- Visit the Reservations page
I have an approved Event or meeting scheduled- Complete the Facilities Request Form, (only if needed).
I have an approved "Long Sanctuary" event- Complete the Audio Ministry Form, (only if needed).
I have an approved "Long Sanctuary" event- Complete the Presentation Ministry Form, (only if needed).
I need a TV, VCR, DVD player or a cassetter player- Complete the Facilities Request Form.
I want to run an announcement- Complete the Announcement Request Form.

I'd like programs or posters made and/or printed- Contact the Graphic Specialist, Louise Alexander
Do I need to directly contact ministries to confirm support? YES you do.
I've contacted the Publishing Ministry for programs, brochures, posters if needed.
Have I arranged for hard copies? (Paper announcements, programs, etc.)
Contacted any group that will be affected by your program? (e.g., Sunday School teachers, if their rooms will be disrupted.)
Do I need to contact the Finance Committee, if an offering is taken? YES you do!
